$50 deposit is required in order to hold your registration. We won't collect that amount until we confirm dates with you. I will send you an invoice to pay if you choose to do online payment.

Usage Agreement *

By checking this box, I as the responsible leader of the group mentioned on this form, am acknowledging agreement of the rules of Mandaville Camp and Retreat Center. I will assume responsibility to clearly communicate this information to my group members. The Mandaville staff will ensure that the facilities, grounds, and program details are in good order and readiness prior to and throughout the guest group's event experience. Should unforeseen and uncontrollable circumstances arise that prevents this agreed-upon event from taking place, both parties will take full responsibility to inform the other party. We agree to not go into staff houses, destroy or break any property. Any damage done by our group will be promptly reported and paid for by our group. We will not use any alcohol or illegal drugs on camp property.
